What you’ll doEvaluate patients in their homes, including strength, mobility, gait, range of motion, pain, and fall risk, and develop individualized, physician‑directed plans of care.
Implement and progress evidence‑based treatment plans to improve strength, balance, flexibility, functional mobility, and safety while reducing pain and risk of injury.
Select and train patients and caregivers on appropriate assistive devices and home exercise programs tailored to the home environment.
Collaborate closely with physicians, the Director of Nursing, and the home health team; provide timely communication on changes in condition and response to treatment.
Complete clear, timely documentation including evaluations, visit notes, progress summaries, and discharge planning, in alignment with regulatory and agency standards.
Treat clients, families, and colleagues with professionalism, courtesy, and respect at all times.
What we’re looking forAt least two (2) years of PT experience; prior home health or community‑based experience strongly preferred.
Strong clinical judgment, excellent communication skills, comfort working independently in the home setting, and dedication to HIPAA compliance and client confidentiality.
