Family Practice Nurse Practitioner; Per-Diem

– that allow clients at all stages of recovery or rehabilitation to reclaim health and well-being.

• Conduct history and physical examinations for clients.
• Review laboratory results and monitor blood levels.
• Handle sick calls and prepare discharge plans, including physician orders and prescriptions if applicable.
• Provide support and resources to the adult residential program and detox unit.
• Coordinate with primary care physicians and psychiatrists as necessary.
• Collaborate with nursing staff to ensure medications are cost-effective post-discharge and complete prior authorization requests.
• Perform other duties as assigned.

• Active Nurse Practitioner license to practice in General/Family Medicine (must possess either an FNP + Family Practice license or a CRNP + Family Practice license).
• Minimum of 2 years of experience in Addiction Medicine.
• Availability for in-person shifts Monday - Friday, with on-call duties via phone or virtually.
• Strong collaboration and communication skills.
• Proven ability to work effectively in a team environment.
