CSAP Program (Crisis Stabilization and Assessment) Hammonton is a short-term intensive treatment home for adolescents with neurodevelopment/psychiatric/medical needs.
Our Transition Specialist will become a part of our multidisciplinary team (Psychiatrist, Board Certified Behavior Analyst, Residential Counselors) that provide an array of therapeutic services to individuals in our care. We provide the highest quality of care and have a commitment to the professional growth and clinical development of our team members.
Transition Specialist Duties- To assist youth and their families in identifying, accessing and receiving appropriate mental health and other services in a timely, efficient and effective manner.
- Participate in the assessment, evaluation and treatment to clients and families under supervision of a licensed social worker.
- Develop individual services plans in conjunction with collaborative agencies, staff, guardians and clients. Participate in counseling services to client, treatment home parents and natural parents. Provide group therapy weekly. Client-centered advocacy, assessing and assuring that quality care is given.
- Provide referral and linkage to ancillary and or specialized treatment services and interventions. Represent the clients on the agency’s behalf on screening, placement, court appearances and any other issues.
- Bachelor's with 3 or more years of relevant experience with individuals with I/DD. Or an unlicensed Master's level with 1 year experience.

EducationBachelors or better.
Experience3 or more years of relevant experience with individuals with I/DD Case Management.
