This job functions autonomously to deliver physical therapy treatment, either through referral or direct to consumer. The incumbent evaluates patients, develops treatment plans, assesses progress and monitors treatment goals. They ensure patient needs are met through access to care, patient experience and quality of care metrics, and assist in program development and enhancement to meet patient and community needs.

• Perform evaluations and provide treatment programs to facilitate patient-specific goals.
• Assess patient response to treatment and recommend plan changes as appropriate.
• Maintain patient records according to documentation standards, policies, and procedures.
• Achieve established productivity levels set by administration, including access to care, patient experience and quality.
• Assist in program development and enhancement to meet patient and community needs.
• Adapt behavior to the specific patient population, respecting privacy, introduction style and communication preferences.
• Perform other related duties as assigned.

• Proficiency in using computers, software and web-based applications.
• Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
• Excellent judgment and decision-making skills in order to deal with a variety of patients and diagnoses.
• Strong interpersonal skills.
• Ability to remain flexible with fluctuating schedules based on patient demand.

Required – Bachelor’s degree in physical therapy.

Required – None.

• Current physical therapy license in the state of practice or eligibility for licensure by examination or reciprocity from another state.
• Basic Life Support (BLS) from the American Heart Association.

Very Heavy Work – Exerting in excess of 100 pounds of force occasionally, or in excess of 50 pounds of force frequently, or in excess of 20 pounds of force constantly to move objects. Physical demands may be in excess of those for Heavy Work. Routine duties require exposure to blood, body fluid and tissue and a risk for communicable diseases.

There may also be exposure to hazardous medications or waste.
