Accepts and processes referrals for children ages birth to three from a variety of sources, including parents, physicians, early childhood professionals, social service agencies, and information and referral systems.
Initiates contact with families to schedule and conduct initial interviews and home visits, while building rapport and explaining the Early Intervention (EI) process.
Collects and reviews pertinent developmental, medical, and social-emotional information to support eligibility determination.
Completes all required developmental evaluations and assessments using standardized tools and team-based input.
Identifies strengths, needs, and areas of concern in collaboration with families and caregivers.
Supports families in understanding their rights and the benefits of Early Intervention services.
Works closely with a multidisciplinary team and community providers to ensure coordinated services.
Facilitates the development, implementation, and regular review of the Individualized Family Service Plan (IFSP), ensuring family priorities and outcomes are addressed.
Collaborates with service providers to monitor child progress, adjusts strategies as needed, and maintains accurate and timely documentation in accordance with state and federal regulations.
Provides ongoing visits in the home.
QUALIFICATIONS- Bachelor's degree or Master's degree in social work, family studies, early childhood education or equivalent or combination of education and work experience or RN two or four year degree or LSW.
- A minimum of two (2) years' experience in working with or providing services to children and families preferred. Familiarity with agencies serving at-risk populations and a demonstrated ability to work cooperatively with local agencies.
- Training in the field of early childhood disabilities and related development/learning disorders and developmental evaluations of infants and young children.
- Experience or knowledge in writing Individual Family Service Plans (IFSP).
- Knowledge of agencies/community resources, public schools, including preschool placement procedures for special education placement in local schools and Head Start.
- Willingness/ability to work as a team member.
- Excellent organizational skills and ability to multitask.
- Demonstrates respect for individual differences.
- History of good job attendance.
- Valid driver's license with good driving record and daily access to private insured vehicle during work hours.
- Ability to work a flexible schedule to meet the needs of families.
- Good communication skills in verbal and written.
- Must pass criminal background checks and be free from communicable disease.
- Proficiency in Spanish-written and verbal preferred.
Salary and work year as established by the Governing Board. The employee shall remain free of any alcohol or non-prescribed controlled substance use in the workplace throughout his/her employment in the District.
