We are seeking a Licensed Occupational Therapist (OT) to join our team this role, you’ll provide comprehensive occupational therapy services including evaluations, treatment planning, ongoing reassessments, and discharge planning while collaborating with residents, families, and healthcare professionals. You’ll develop individualized care programs, modify treatments as resident needs change, and ensure optimal functional outcomes. Responsibilities- Administers occupational therapy in compliance with Federal, State, local, and Community First Solution’s standards, policies, procedures, and guidelines.
- Implements strategies and interacts with staff members and other healthcare professionals to ensure efficient and quality care to residents.
- Interacts with residents, families, support groups, staff member, and other health professionals to ensure maximum rehabilitation to the resident while serving as resident advocate.
- Completes Occupational Therapy evaluations and treatments, plan of care, assessments, and discharge orders in a timely manner.
- Must comply with OSHA standards and ensure the safety of the residents under their care at all times.
- Co-signs all medical record documentation written by Certified Occupational Therapy Assistants (COTA) and provides ongoing guidance and support to COTAs.
- Analyzes data and provides recommendations for residents’ care and home assessments.
- Adheres to the American Occupational Therapy Association’s Code of Ethics, infection control policies, professional therapeutic services, and Community First Solutions standards, policies, procedures, and guidelines.
- Adheres to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
- Adheres to all policies and procedures of the organization regarding the use of technology and technology-based information.
- Responsible for welcoming and on-boarding new employees.

- All other duties as assigned and/or appropriate to the position.
Education: Bachelor’s degree from an accredited Occupational Therapy Program (Master’s degree preferred).
Licenses or other required certifications: Current Ohio License in Occupational Therapy. CPR certification.
Experience: At least two years’ experience, preferably in a long-term health care setting.
