Responsibilities
  • All job tasks and functions must be performed under the auspices of AWARE Inc. Care Values, Principles, and Unconditional Care Philosophy.
  • Provides direct clinical services to clients individually, in a group setting and on a family basis in the office and in the community‑at‑large to teach behavioral symptom management techniques to alleviate and manage symptoms not reduced with medication.
  • Promotes personal growth and development by assisting clients to adapt to and cope with internal and external stressors.
  • Provides care coordination for an assigned group of clients including coordination and monitoring of activities of the individual treatment team. Assumes secondary responsibilities for developing, writing, implementing, evaluating and revising overall treatment plans as clients’ needs change; educates and supports clients’ families and advocates for clients’ rights and preferences.
  • Assesses the needs of clients and contributes to written assessments that describe levels of functioning for the client in a number of specific areas.
  • Provides benefits counseling (i.e., SSI, veteran’s benefits, housing) and supports access to financial and medical supports.
  • Provides vocational counseling to assist clients in meeting employment and/or educational.
  • Assists and supports clients to carry out personal hygiene and grooming tasks.
  • Provides nutritional education, meal planning, grocery shopping and food preparation skills.
  • Assists clients in finding and maintaining safe and affordable housing by negotiating with landlords and housing development agencies; provides education with cleaning and procuring necessities including ongoing supportive services.
  • Under the direction of nursing staff, assists clients with their medications through phone prompts, in‑person prompts and utilizing medication lockboxes.
  • Performs on‑call duties for crisis stabilization needs on a rotating basis with other team members which includes nights and weekends.
  • Maintains individual charts per AWARE policy, practice and supervisory directive.
  • Must demonstrate – on a consistent basis – the willingness and ability to productively utilize work time in order to meet the needs of the individuals being served as directed, with the understanding that specific fiscal goals (billing, case load number maintenance, weekly) must be met on a monthly basis.
  • Develop a crisis plan with the caregiver that identifies a range of potential crisis situations with a range of corresponding responses including direct and telephonic response 24/7.
  • Records notes, billing units, assessments, referrals, and other data entry assignments via AWARE’s Electronic Healthcare Record System.
  • Upon hiring, the Mental Health Professional will undergo training and will be expected to pass testing specific to the position.
  • Performs other duties as assigned.
