Social Worker - Hamilton, NJ
Location: RWJUH Hamilton Hospital, One Hamilton Health Place, Hamilton, NJ 08619
Department: Case Management
Shift: Day – Per Diem (requires at least one weekend a month)
Pay Range: $37.50 – $47.21 per hour
Job OverviewThe Social Worker collaborates with the healthcare team to facilitate patient treatment and discharge planning. Responsibilities include assessment, coordination, and education of patients, caregivers, and team members to achieve treatment goals from hospitalization through post‑hospitalization care. The role ensures continuity of care for patients with complex post‑acute needs and maximizes family resources within their social and financial frameworks.
Qualifications (Required)- Graduation from an accredited graduate school of Social Work with an MSW degree
- One or more years of experience in an inpatient acute hospital setting
- Case Management certification
- State licensure required by the NJ State Board of Social Work Examiners
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW); qualification depends on experience and coursework
- Complete initial psycho‑social assessments and provide psychotherapeutic counseling to individuals, families, or groups as needed
- Collaborate with interdisciplinary team, manage complex discharge plans, and maintain relationships with internal and external resources
- Coordinate referrals to community agencies and entitlement programs, advocating for patients and families
- Provide crisis interventions and psychosocial counseling to support adaptation to illness and discharge
- Arrange placement in inpatient psychiatric units, intensive outpatient mental health, or substance abuse programs as appropriate
- Assist patients and families in locating outpatient resources and assess cultural, religious, emotional, and language barriers
- Document findings and communicate information to the multidisciplinary team, ensuring accurate medical record keeping
- Develop discharge plans in collaboration with patient, family, and health‑care team, and coordinate with physicians and medical staff on patient needs
- Make referrals to DCPP, Adult Protective Services, and other agencies, and assist with transfers or discharges to appropriate levels of care
- Notify supervisors of difficult placement concerns and collaborate with Risk Management and legal staff on guardianship or legal issues
- Accurately complete all transfer and discharge summary forms
- Perform other duties as assigned
