POSITION PURPOSE

SUMMARY:


The Community Outreach Coordinator provides individualized support to parents who experience intellectual and developmental disabilities, helping them to meet their identified parenting goals. They also provide group-based parenting education to support healthy child development and relationships. The Coordinator collects data and develops community connections to help build long-term program sustainability.

The schedule will vary based on program participant needs. Evening and weekend work is required on an as-needed basis. Shifts include a ½ hour paid meal break where applicable. The Program Supervisor sets the hours of work; some flexibility is required.

Accountabilities
  • Carry out intake processes, including assessment and interview, for each program participant
  • Provide individualized training and education to help participants achieve their identified parenting goals
  • Document training procedures and track individual progress
  • Develop, promote, and facilitate support groups to address participant needs
  • Provide community referrals and service navigation support
  • Provide education based on evidence-informed best practices in child development
  • Respond to emergencies and other serious occurrences in a timely manner per policy
  • Assist individuals in building natural supports to foster community engagement
  • Work with participants, family support networks, and other developmental service agencies to build sustainable supports
  • Act in accordance with anti-oppressive practice and inclusion, equity, and diversity per agency policy
  • Complete risk assessments and introduce health & safety controls as appropriate
  • Accurately collect program data and assist with program evaluation
  • Demonstrate understanding of the Child and Family Services Act and its implications for parents with cognitive limitations
  • Abide by confidentiality limits and the duty to report; consult with the Program Supervisor as appropriate
  • Participate in regular supervision, team meetings, and performance evaluations
  • Ensure upkeep, cleanliness and general maintenance of all work spaces
  • Complete training requirements in alignment with MCCSS and agency standards
  • Represent the organization in a respectful and professional manner, reflecting The Salvation Army’s mission and values
  • Other duties as assigned by the Program Supervisor, Program Director, or Executive Director
Relationship Management
  • Internal:
    Regular interaction with Program Supervisor and other team members; regular interaction with program participants and their families
  • External:
    Primary care support team; community partners
  • Managerial/Technical Leadership:
    This position reports directly to the Program Supervisor
  • Financial and Materials Management:
    Uses organizational assets; does not process financial transactions or have budget responsibilities
Working Conditions
  • Working environment in the Hamilton community, participant homes, and in office
  • Evenings and weekends may be required to meet participant needs
Education, Experience, And Qualifications
  • Completion of a formal post-secondary program of at least two academic years in Developmental Services Worker, Early Childhood Educator, Psychology, or equivalent
  • Current First Aid & CPR with Defibrillation training or willingness to obtain upon hire
  • Non-Violent Physical Crisis Intervention Certification or willingness to obtain upon hire
  • Provide an original copy of Criminal Records Check with vulnerable sector screening
  • Must have valid Ontario Class G or G2 Driver’s license and access to a vehicle
