FT Rapid Mobile Case Manager Diversion - Mary's Place

HOURS - 37.5 hours per week.
Tuesday to Saturday OR Sunday to Thursday (2 positions) – rotation of 2 weeks 9am‑5pm and 2 weeks 1pm‑9am.

How You Will Make An Impact

The successful candidate will extend our mission to residential and non‑residential clients through respectful and appropriate housing support and diversion services. This will include assessment, diversion planning, case management, crisis intervention, and supportive counseling. The incumbent must be able to work independently in a challenging, values‑based environment.

  • Provide brief mobile case management and diversion services to individuals at imminent risk of homelessness, helping them avoid the need to access emergency shelter.
  • Work collaboratively with shelter staff to complete assessments for shelter eligibility, plan intakes, and make referrals to overflow spaces as needed.
  • Partner with various agencies to coordinate assets and services, ensuring that service users receive individualized housing and support needs.
  • The work occurs in the office, in emergency shelters, and in the community.
  • Accompany service users to community and housing appointments and apartment viewings.
  • Assist service users in a hands‑on capacity.
  • Observe all health and safety rules and comply with the Occupational Health and Safety Act and its regulations.
What You'll Bring To Our Team
  • Post‑secondary education in Social Services or a related field is required.
  • Minimum 1‑year related experience or an equivalent combination of education, training and experience in Social Services.
  • Preferred:
    Minimum 1‑year experience working in community outreach or emergency shelter settings.
  • Understanding and utilization of person‑directed, anti‑racism/anti‑oppression and feminist frameworks; comprehensive knowledge of chronic homelessness, poverty, abuse, mental health and addictions.
  • Commitment to, and experience working within, a trauma‑informed, harm reduction and housing‑first framework.
  • Excellent interpersonal and conflict resolution skills, strong verbal and written communication, time management, organization and ability to prioritize workload.
  • Ability to work cooperatively and collaboratively within an interdisciplinary team.
  • CPR certification or willingness to be certified – required.
  • Valid G license and willingness and ability to drive the company vehicle – required.
  • Fluency in a second language is an asset.
