Hamilton Health Sciences is a hospital system of 18,000 staff, physicians, researchers and volunteers that proudly serves south central Ontario residents. We also provide specialized, advanced care to people from across the province.
Located in central Hamilton, St. Peter’s Hospital (SPH) provides complex continuing care across various inpatient program including palliative care, restorative rehabilitation, complex medical and behavioral care. Our Medically Complex Care unit is a large 63-bed unit. Our interdisciplinary team provides care for adult patients following their acute phase of care, who have multiple or complex medical needs which may include tracheostomies and complex wounds.
Medically Complex patients require interventions to obtain medical optimization in order to prepare them for discharge back to the community.
The longer length of stay allows nurses to get to know their patients as persons and form professional working relationships with both the patient and their family. Educational opportunities are plenty, and since we host many students from several disciplines, there are opportunities to become a preceptor. We offer a friendly team environment.
The Registered Practical Nurse on Medically Complex provides person-centred care and functions within the full scope of practice of the RPN by gaining the knowledge and skills to build upon foundational knowledge to promote the best possible psycho-social, emotional, and physical outcomes for patients on Medically Complex. The RPN collaborates with the RN using the College of Nurses of Ontario’s three-factor framework, and works closely with other nurses and members of the interdisciplinary team, and may assign tasks and elements of patient care to unregulated care providers.
The RPN is supported by the Clinical Manager and other nurses including Registered Nurses, Nursing Case Managers, Nurse Practitioner, Clinical Nurse Specialist, and Education and Development Clinician.
