Job Title:
Wound Care Nurse

Position Type: Full-Time

Reports To: Director of Nursing / Clinical Services

Job Summary

We are seeking a dedicated and experienced Wound Care Nurse to provide specialized wound assessment and treatment for residents in our Skilled Nursing Facility. The ideal candidate will deliver high‑quality clinical care, develop individualized wound care plans, monitor healing progress, and collaborate with the interdisciplinary care team to ensure optimal outcomes.

Key Responsibilities
  • Perform comprehensive wound assessments, including documentation of wound characteristics and changes.
  • Develop, implement, and revise individualized wound care treatment plans based on current best practices and physician orders.
  • Perform wound treatments such as dressing changes, debridement (as appropriate), and application of advanced wound care products.
  • Monitor, document, and communicate wound healing progress in the electronic health record.
  • Educate residents, families, and facility staff on wound care protocols and prevention strategies.
  • Collaborate with physicians, dietitians, therapists, and other staff to ensure coordinated, high‑quality care.
  • Participate in weekly wound care rounds and care plan meetings.
  • Maintain wound care supplies and ensure proper use of equipment.
Qualifications

Required:

  • Current Registered Nurse (RN) license (state of practice).
  • Minimum of 2 years of nursing experience — preferably including wound care or long‑term care experience.
  • Basic Life Support (BLS)/CPR certification.

Preferred:

  • Wound care certification (e.g., WOCN, WCC, CWCN).
  • Experience with pressure ulcer, diabetic ulcer, and complex wound management.
  • Strong assessment, communication, and interdisciplinary collaboration skills.
