OverviewThis role exists to make care safe, coordinated, and humane as Anchor grows. The RN Care Coordinator applies clinical judgment, continuity, and coordination so patient care remains predictable in a fast-moving ambulatory environment.
Responsibilities- Stabilize care delivery through early risk detection and appropriate escalation
- Create continuity across visits and handoffs so care feels coherent rather than fragmented
- Translate provider intent into reliable execution across follow-ups and next steps
- Reduce operational friction by coordinating with MAs, case management, and operations
- Strengthen patient understanding and agency through clear, respectful communication
- Maintain defensible documentation that supports quality and regulatory standards
- Patients experience care as coordinated, safe, and predictable
- Providers trust that clinical intent is executed accurately
- Medical Assistants and staff have clarity and support
- Active CT Registered Nurse license in good standing
- 1–3 years of clinical experience within RN scope
- Strong clinical judgment and prioritization skills
- Effective communication with patients, providers, and staff
- Comfort operating in a community health and ambulatory environment

SalaryThe salary range for this role is $86,000 - $95,000 annually.
Compensation within this range is determined based on relevant experience, licensure, skills, scope of responsibility, and internal equity. The full range reflects the potential for growth within the role; most new hires are typically placed between the minimum and midpoint of the range, depending on qualifications. Anchor Health maintains structured salary bands and reviews compensation periodically to ensure alignment with market conditions, organizational needs, and individual performance.
