NEPA Community Health Care is seeking an experienced LMFT to join our care team in Hallstead, PA! Providers have the option to work either four 10-hr or five 8-hr shifts. Salary negotiation will be based upon experience and credentials;

This role provides therapeutic interventions to remove psychosocial barriers to health and wellness; collaborates with primary healthcare team, the patient/family and community liaisons to assure patients’ psychosocial and socioeconomics needs are addressed wherever possible; participates in monthly staff meetings and physical/behavioral health integration efforts; and may participate in regional human services collaboratives as health center representative with a focus on improvement of quality of life in the health center service area.
