Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge. Ideal candidates thrive in interdisciplinary environments and are committed to improving patient outcomes through holistic, evidence-based care.
Key Responsibilities:
- Coordinate psycho-social support for patients during hospitalization.
- Develop and implement personalized discharge plans.
- Assess patient needs related to care coordination, resources, and services.
- Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction.
- Facilitate access to community resources to support recovery and continuity of care.
- Ensure compliance with hospital policies, regulatory standards, and evidence-based practices.
Qualifications:
Education:
Bachelor’s degree in Social Work required
Experience:
Minimum two years in acute care, SNF, physician offices, or community health settings
Licensure/Certification:
LBSW license in Maryland required
Must obtain care/case management certification within five years of hire
Skills & Abilities:
- Strong communication and multitasking skills
- Familiarity with EMRs and ability to navigate electronic medical records
- Knowledge of disease processes, regulatory requirements, and evidence-based care
- Understanding of community resources, provider networks, and the revenue cycle
- Ability to build relationships across clinical and community settings
