Job Description

Responsible for providing continuous observation and appropriate documentation on activity of an assigned patient who has been identified as having increased risk for injury.

• Provides ADL's and protects patients in sitter and spotter assignments from self-harm and prevents elopements.
• Makes continuous rounds.
• Assists with activities of daily living.
• Assists with bathroom, commode, and incontinence care.
• Monitors intake and output.
• Performs preventive skin care, feeding mouth care, and changes bed linens when necessary.
• Performs other related duties as assigned.

• High School Diploma or equivalent.
• Prior experience in patient care.

Hackettstown Medical Center has been providing care to the community since 1973, primarily serving Warren, Sussex and Morris Counties in New Jersey.
