Program Summary
:

Center For Family Services is seeking experienced and professional Clinician/Program Supervisor for the Crisis Diversion Home Program. This is a 24-hour residential group home for adults who recently experienced a mental health crisis. The Crisis Diversion Home provides a nurturing, supportive, and structured environment where our consumers can develop trust, learn to manage their illness and live independently.

Job Type/Status: Full time, Monday
- Friday with on-call responsibilities

Duties and Responsibilities:

  • Creates and maintains a wellness and recovery-oriented environment by using person-centered planning, language and documentation, and by insuring that services provided promote wellness and recovery outcomes.
  • Ensures direction and evaluation of CDH team members and participates in the hiring and termination process of CDH staff; as needed, provides education and technical assistance to team members regarding recovery and wellness; works with the team to develop relevant strategies, such as Wellness Recovery Action Plans (WRAPs), instilling hope, promoting choice and empowerment, and developing social and spiritual supports; serves as a resource for the team for wellness and recovery strategies.
  • Ensures that relevant data is collected for contract monitoring, quality improvement, and other purposes as needed; insures that data is used to improve services provided.
  • Ensures coordination and integration of CDH services with other Center for Family Services programs and other community programs/agencies in a manner that benefits consumers served.
  • Provides on-call coverage on a rotating schedule.
  • Provides direct service to CDH consumers and their family members; works with the consumer and family to plan for transition out of the CDH; works with consumer, family, and CDH APRN to develop an initial treatment plan; works with consumer and family members to develop and implement a Wellness Recovery Action Plan (WRAP) and to develop treatment/WRAP plan adherence supports using consumer peer specialists as indicated;

    conveys hopefulness toward people served and their families.
  • Participates in quality improvement activities within the program and participates in agency-wide quality improvement activities, as directed.

REQUIREMENTS:

  • Master's degree in Social Work or allied field of study, licensed in NJ to provide clinical services
  • 2 years of experience working with the serious and persistent mentally ill population including the dually diagnosed (Co-occurring) population
  • Valid driver's license with acceptable driving record.
  • LCSW with supervisor's credentials and CADC preferred.
