- Assessment, Planning, Implementation, and Evaluation of patient care.
- Collect comprehensive data pertinent to the patient's health or the situation.
- Perform age and disease‑specific assessments independently in a systematic manner focusing on physiological, psychological, and cognitive status.
- Recognize clinical and diagnostic status changes and identify variables in patient/family education needs based on age, disease, and culturally specific learning differences.
- Develop and coordinate a plan of care that is patient/family focused and prioritized, encompassing the continuum of care.
- Communicate, coordinate, and implement the plan of care with the healthcare team and the patient/family.
- Provide ongoing communication with patient/family and nursing colleagues regarding patient status and treatment plans.
- Implement age and disease‑specific care in an organized and timely manner.
- Provide or coordinate the patient/family education as needed and ensure understanding.
- Perform according to established protocols in clinical situations, including documenting events.
- Advocate appropriately and demonstrate evidence‑based nursing practice, complying with standards.
- Perform continuous point‑of‑care electronic documentation of assessments, care plans, interventions, and evaluations.
- Administer medications utilizing the newest bar‑scanning and documentation technologies designed to ensure safe delivery.
- Participate in multidisciplinary discharging of patients to ensure safe transition and continuity of care.
- Evaluate progress toward attainment of outcomes and utilize critical thinking skills to achieve desired patient outcomes.
- Initiate interventions related to patient’s change in condition and unexpected responses to care.
- Identify ethical issues and bring them to the attention of other team members.
- Respond to opportunities to enhance patient satisfaction.
- Acquire and maintain competencies, certifications, and continuing education to meet facility accreditation or individual licensing requirements.
- Adhere to the American Nurses Association Code for Nurses and HMH's Organizational and Managerial competencies and standards of behavior.
- Lift a minimum of 35 lbs., push and pull a minimum of 10 lbs., and stand for a minimum of 6 hours daily.
- Perform any other duties and/or projects as assigned.
- Minimum of an Associate in Nursing or Diploma of Nursing degree.
- BSN in progress and/or willingness to complete a BSN within three (3) years of hire.
- Current and valid Registered Nurse license to work at the assigned HMH location by the date of hire.
- BLS/CPR certification by the American Heart Association required within 60 days of hire.
- Six (6) or more months of relevant nursing experience or active participation in Hackensack Meridian Health's Nurse Residency Program.
- Ability to learn quickly, adapt to changing patient needs, and demonstrate a strong sense of accountability and teamwork.
- Preferred experience:
PACU, ED, Telemetry, Critical Care, or National Certification in a specialty area.
Minimum hourly rate: $47.64.
