Patients benefit from the Ambulatory Pharmacy Navigator program, which ensures quick and efficient delivery of discharge and specialty medications, enabling smooth hospitalization transitions and immediate start of therapy.
Responsibilities- The Navigator performs all duties of an Ambulatory Pharmacy Technician and additional tasks listed below.
- Provides an initial greeting to admitted patients or clinic patients and explains available pharmacy services.
- Performs prescription order entry duties as needed.
- Obtains patient authorization to process prescriptions through ambulatory or specialty pharmacy.
- Collaborates with nurses, discharge planners, case managers, social workers, physicians, APNs, PAs, medical residents, unit clerks and other teams to ensure patients scheduled for discharge or clinic appointments receive the medications on time.
- Ensures HMH Pharmacies are selected by patient choice for medication services.
- Determines necessary information sources for processing prescriptions, such as insurance coverage.
- Identifies and troubleshoots medication‑access issues related to insurance, financial or coverage and resolves them so patients can receive needed medications.
- Adheres to HMH Pharmacy and federal/state privacy and compliance policies to protect patient confidentiality.
- Ensures prescription fulfillment by in‑house pharmacy based on priority needs to allow for timely discharge or start of specialty medication.
- Coordinates patient pickup or delivery of prescriptions to patient care areas or home.
- Ensures patients receive an offer for pharmacist counseling and distributes pharmacist contact information for any post‑discharge questions.
- Facilitates distribution of medication‑related educational materials and coordinates counseling with staff pharmacist as needed.
- Assists ambulatory pharmacy staff and performs any other assigned duties.
- Maintains a 'safety first' work ethic and reports medication errors or variances as required by HMH Pharmacy Policy.
- Other duties and/or projects as assigned.
- Adheres to HMH organizational competencies and standards of behavior.
- High School diploma, GED, or equivalent program.
- Excellent written and verbal communication skills.
- Proficient computer skills, including Microsoft Office and/or Google Workspace.
- Retail/Specialty pharmacy experience.
- Strong knowledge and experience with PBM claims and coverage.
- Pharmacy Technician License.
- PTCB or equivalent certification required within 180 days of employment, except for:
- Hired prior to January 2024.
- Actively enrolled in an ACPE‑accredited school of pharmacy or other allied health program, or hired before 01‑01‑24.
Minimum hourly rate: $26.71. The posted rate is a good‑faith estimate of the minimum base pay at the time of posting, per the New Jersey Pay Transparency Act. Total rewards may include bonuses, shift differentials and a comprehensive benefits package for full‑time and part‑time workers.
