• Identify, resolve, and prevent drug‑related problems.
• Recommend and monitor drug therapy.
• Participate in the department’s drug formulary review process, including preparation of drug monographs for review by the Pharmacy and Therapeutics Committee.
• Participate in drug guideline and order set development.
• Participate in drug research related activities.
• Participate on various Medical Center committees and service lines as dictated by specific area(s) of expertise (e.g., cardiology, oncology, pediatrics).
• Provide drug information to various health care providers.
• Serve as a resource to the department in area(s) of expertise by helping to ensure that drug treatment plans and drug therapies are safe and effective.
• Participate in and assist in conducting educational programs for physicians, nurses, pharmacists, pharmacy students, and other health care providers.
• Participate in pharmacy student rotation program.
• Participate in the Pharmacy Practice Residency program as a preceptor and member of the Residency Advisory Committee.
• Assist with staffing as deemed necessary by the Director of Pharmacy.
• Identify the needs of the patient population served and modify and deliver care that is specific to those needs (age, culture, language, hearing/visually impaired, etc.). Communicate with the patient, parent or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).
• Adhere to the standards identified in the Medical Center’s Organizational Competencies.

• Graduate of an ACPE accredited school of pharmacy with a minimum of a Pharm.
  
  D. Degree.
• Completion of a clinical pharmacy residency program or 5 years of hospital pharmacy experience with a minimum of three years in clinical practice.

• Advanced training in a specialty area (e.g., oncology, pediatrics, cardiology, infectious disease, etc.).

• Registered Pharmacist License.

Lifts a minimum of 10 lbs., pushes and pulls a minimum of 40 lbs. and stands a minimum of 4 hours a day.

Minimum rate of $ Annually. The posted rate of pay is a reasonable good‑faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market‑competitive total rewards package.

Some jobs may also be eligible for performance‑based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
