The Outreach Nurse Navigator, JTCC guides patients through the health care system by conducting community outreach and education and promoting cancer preventive screenings for the JTCC catchment area. The navigator works in partnership with Outreach, Research and JTCC clinical teams to proactively identify and provide for patients' needs. This role helps establish and maintain JTCC's reputation in the community and trust with patients, and is often the first contact to ensure patients are informed of critical cancer screenings and are not lost to barriers or unable to receive services.
Responsibilities- Tracks screening and outreach activities, interventions and outcomes in a database.
- Facilitates relationships with community partners and service providers. Includes providing and promoting cancer-related health education (including clinical trials), identifying and addressing patient barriers, facilitating appointments, conducting eligibility screening and ensuring patient compliance for preventive cancer screenings.
- Provides resources to help patients with social issues.
- Assists patients with making follow-up appointments and filling prescriptions.
- Helps patients fill out applications for benefits.
- Analyzes health system data to support the program and identify patients at high risk for poor outcomes.
- Explains and enrolls patients into the program, as a supplement to CHW-initiated enrollments.
- Coordinates program activities including assistance with CHW recruitment, hiring and training.
- Analyzes data collected by CHWs and generates reporting of program metrics to key stakeholders.
- Maintains and updates project plan in support of project operations.
- Provides administrative support for policy documentation processes.
- Works with other CHWs and staff to create a directory of community resources.
- Prepares reports and documents as needed or requested.
- Other duties and/or projects as assigned.
- Adheres to HMH Organization competencies and standards of behavior.
- BSN degree from an accredited college or university
- NJ State Professional Registered Nurse License
- Excellent organization skills
- Excellent written and verbal communication skills
- Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms
- Ability to carry out written and oral instructions
- Ability to exercise judgment in the application of professional services
- Experience with data collection/management/analysis
- Experience serving in low-income communities preferred
