Position: Part-Time Bone Health and Osteoporosis Specialist (Nurse Practitioner / Physician Assistant)

Overview

We are seeking a dedicated and detail-oriented Advanced Practice Provider to join our Rheumatology Bone Health Clinic. In this role, you will serve as the primary clinical provider for patients at risk for or diagnosed with osteoporosis and related metabolic bone diseases. You will be responsible for the full spectrum of bone health management, from initial diagnostic screening and imaging to the development of long-term pharmacological and lifestyle treatment plans aimed at reducing fracture risk.

Job Type: Part-time

Key Responsibilities
  • Clinical Assessment & Diagnosis:
Conduct comprehensive evaluations of patients with low-impact fractures, low bone density, or high risk for osteoporosis.
  • Order and interpret diagnostic tests, including DXA scans (Dual-energy X-ray Absorptiometry), Vertebral Fracture Assessments (VFA), and specialized metabolic bone lab panels.
  • Utilize FRAX (Fracture Risk Assessment Tool) scores to quantify patient risk and determine treatment thresholds.
Treatment & Care Management
  • Develop individualized treatment plans for the prevention and management of osteoporosis.
  • Prescribe and manage bone-modifying medications (e.g., bisphosphonates, RANKL inhibitors, and anabolic agents).
  • Coordinate with the Infusion Center team on the administration of injectable or infusible bone health medications.
  • Perform routine follow-up assessments to monitor treatment efficacy and adjust protocols as needed.
Patient Education & Lifestyle Counseling
  • Counsel patients on nutritional strategies, specifically regarding Calcium, Vitamin D and protein intake.
  • Advise on physical therapy and weight-bearing exercise programs and fall prevention strategies to mitigate injury risk.
  • Provide clear education on the "silent" nature of bone loss to ensure long-term medication compliance.
Collaborative Care
  • Act as a bridge between orthopedic surgeons, rheumatologists and primary care providers to ensure continuity of care post-fracture.
  • Maintain accurate and timely documentation within the EMR system.
Requirements
  • Education: Master’s degree from an accredited Physician Assistant program or Advanced Nurse Practitioner program
  • Licensure: Current PA / APRN License and applicable Board Certification.
  • Certification: BLS/ACLS Certification required / Fracture Liaison Certification preferred
  • Experience: Experience in Orthopedics, Rheumatology, or Endocrinology preferred, but we are willing to train the right candidate with a strong interest in metabolic bone health.
Skills & Competencies
  • Strong analytical skills in interpreting imaging and laboratory data.
  • Excellent communication skills with an emphasis on empathetic patient education.
  • Ability to work independently while collaborating effectively within a multi-disciplinary healthcare team.
