Position Overview

Case Manager - In-Circle Program

Responsibilities
  • The in-Circle Program Case Manager is responsible for assessing service needs and linking the child and family with a range of wraparound services based on the client’s individualized needs (social, educational, vocational, recreational, and related supports).
  • Serve as an adult role model who collaborates with the family to create a supportive environment addressing basic needs through referral, education, and skill development.
  • Collaborate with the CPS caseworker to ensure effective implementation of services and adjust plans as needs change.
  • Provide comprehensive case management that includes assessment, service plan development, referral/linkage to services, and progress/outcome monitoring.
Job Responsibilities
  • After referral, coordinate with local MCPS staff and contact the family within 24 hours.
  • Assist eligible individuals in gaining access to medical, social, educational, vocational, and other services.
  • Ensure case management services are provided and documented in accordance with program requirements.
  • Monitor participants’ progress and document outcomes.
  • Complete initial case management goals, objectives, and interventions for the case plan in coordination with the in-Circle Program therapist, MCPS, and the family.
  • Facilitate the initial family team meeting with MDHS and subsequent meetings as outlined by the Individualized Service Plan.
  • Review and update the Crisis Management Plan (CMP) with each assigned family at each Family Team Meeting, during service plan reviews, and after crises.
  • Complete weekly ISP reviews and document progress toward service goals.
  • Assess the strengths and needs of referred families and utilize community resources to meet those needs.
  • Act as a positive role model in every interaction with participants.
  • Provide direct case intervention services to children, adolescents, and families in home, school, office, community, and other appropriate settings.
  • Provide and coordinate family-driven, community-based social services.
  • Provide transportation for families in a safe and responsible manner.
  • Conduct satisfaction surveys as assigned and prepare program reports as required.
Qualifications
  • Bachelor’s degree in Social Work or a related field; at least two (2) years of experience working with children and families.
  • Competence in working with children and families in the child welfare system.
  • Competence in completing documentation and reports related to provided services.
  • Computer skills required.
  • Must be at least 21 years of age, have a valid driver license, a good driving record, proof of auto liability insurance, and willingness to use a personal vehicle for organization business.
