Dietitian – Homewood Health Centre

What You’ll Be Doing- Conduct comprehensive nutrition assessments for patients.
- Develop and implement individualized nutrition care plans.
- Provide counseling and education and in‑services to patients, using a trauma‑informed and patient‑centered approach.
- Document as per HHC and college standards, including assessments, interventions, and outcomes in patient medical record.
- Collaborate with the clinical team to provide the best nutritional care for patients.
- Advise and develop custom menus, snacks.
- Promote healthy eating behaviors.

What We’re Looking For- Bachelor’s degree in Human Nutrition and Dietetics from an accredited post‑secondary institution.
- Valid registration with the College of Dietitians of Ontario is required; membership in Dietitians of Canada is preferred.
- 5 years’ previous experience in Older Adult and Eating Disorders.
- Experience with mental health and addictions populations.
- Collaborative, service‑oriented, innovative and accountable.
- Commitment to working within Homewood’s Health and Safety Standards.
- Practice in accordance with the ethical guidelines of the Dietetics profession.
