• Conduct comprehensive nutrition assessments for patients.
• Develop and implement individualized nutrition care plans.
• Provide counseling and education and in‑services to patients, using a trauma‑informed and patient‑centered approach.
• Document as per HHC and college standards, including assessments, interventions, and outcomes in patient medical record.
• Collaborate with the clinical team to provide the best nutritional care for patients.
• Advise and develop custom menus, snacks.
• Promote healthy eating behaviors.

• Bachelor’s degree in Human Nutrition and Dietetics from an accredited post‑secondary institution.
• Valid registration with the College of Dietitians of Ontario is required; membership in Dietitians of Canada is preferred.
• 5 years’ previous experience in Older Adult and Eating Disorders.
• Experience with mental health and addictions populations.
• Collaborative, service‑oriented, innovative and accountable.
• Commitment to working within Homewood’s Health and Safety Standards.
• Practice in accordance with the ethical guidelines of the Dietetics profession.
