Een zorgorganisatie voor jeugdigen in Winschoten is op zoek naar een persoonlijk begeleider. In deze rol ondersteun je jongeren bij het behalen van hun doelen en werk je samen met betrokken instellingen. Je bent verantwoordelijk voor het opstellen van zorgplannen en het rapporteren over de ontwikkelingen. Een relevante HBO-opleiding is vereist evenals goede communicatieve en sociale vaardigheden. Het contract is voor 28-32 uur per week met kans op verlenging bij goed functioneren.
