Support Time & Recovery Navigator
About the RoleAt Practice Plus Group, we put people’s wellbeing first. Our Health in Justice service line is now expanding into non‑Custodial healthcare, delivering a person‑centred approach to assess mental health and neurodiversity requirements and redirect individuals to the most appropriate care channel.
LocationHumberside – South Bank
SalaryThis full‑time position (37.5 hrs per week) offers a salary of £29,264 to £33,510, depending on experience.
HoursMonday – Friday: 9 am – 5 pm
Legal RequirementsCandidates must have legal right to work in the UK. Offers of employment will be subject to additional vetting and security checks.
Responsibilities- Work as part of an extended team across Liaison & Diversion services.
- Liaise with police, probation, courts, legal services, as well as inpatient and community health and social care services.
- Provide daily support to people in contact with children and adults, including hard‑to‑reach communities, to navigate the Criminal Justice System.
- Support access to holistic health needs, including substance misuse, mental and physical health, homelessness, learning disabilities, exploitation, safeguarding, and social care issues.
- Deliver practical support to individuals, maintain a support programme, work with families, advocate and enable access to agencies such as probation services, community organisations, clinics, courts, mental health services, and hospitals.

- Level 3 NVQ/Diploma in Health and/or Social Care or equivalent experience.
- Experience working face‑to‑face with service users with diverse health needs.
- Understanding of multi‑disciplinary and multi‑agency working.
- Excellent communication skills and ability to obtain appropriate consent.
- Knowledge of clinical governance, health and safety.
- Familiarity with evidence‑based practice, health promotion, NICE guidelines.
- Proactive problem‑solving skills.
- Flexible attitude to working arrangements.
- Motivated individually and as part of a team.
- Patient‑centred values and commitment to service user involvement.
