• Providing 1:1 care for client following client’s plan of care at all times while maintaining client’s dignity and independence
• Administering medications, feedings, oxygen, and ostomy care as needed per client’s plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communicating with family, case manager, and physicians, documenting progress and informing about changes in client’s medical condition
• Coordinating or ordering supplies, medications, and feedings as needed to ensure adequate amounts are always on hand
• Ensuring all patient needs are met in accordance with the instructions of the physician

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record, and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home health care experience preferred

Adoration Home Health / Bright Spring Health Services, an affiliated company, has recently acquired certain LHC Group home health and hospice operations. The successful candidate will initially be employed by LHC Group until January 1, 2026. During this period, LHC Group will perform all administrative onboarding activities and offer benefits coverage under LHC Group employee benefit plans. On or before January 1, 2026, employment will transfer to Adoration Home Health / Bright Spring Health Services under the same terms.
