RN Nurse Case Manager

Works in cooperation with the interdisciplinary team to effectively implement the Plan of Care. Provides nursing care in accordance with medical orders and the Plan of Care. Nursing care is provided for patients enrolled in the Hospice and Palliative Care Home Care Program.

• Associate Degree or Bachelors Degree in Nursing. Bachelors degree preferred.
• A minimum of two years experience in a medical surgical environment, home care or hospice program. Hospice and home care experience preferred.
• Ability to assess and respond to the complex needs of terminally ill patients and their families
• Skilled in nursing practice, able to provide emotional support and accept individual lifestyles
• Skilled in pain and symptom management
• Professional, positive attitude required
• Ability to function as part of a team
• Ability to embrace the mission and culture of the organization

• Manage the care of assigned patients by making visits to patients within the service area and providing nursing care which includes patient/family teaching and support
• Communicate regarding the patient's status with physician, team members and other agencies involved in the care of the patient
• Attend team conferences, staff meetings and in-services as assigned
• Document patient/family contacts, nursing care provided and other required documentation in an accurate and timely manner
• Adhere to all regulatory standards and agency policies as set forth
• Coordinate the transition of patients and changes in level of care
• Assist with referrals, admissions and evaluation visits as needed
• Support the overall culture of the organization
• Participate in organization-sponsored events
• Perform other related duties as assigned

Full-time position, 32 hours per week.
