Overview
Provides behavioral health social work services for inpatient and Emergency Care Center (ECC). In coordination with physicians/psychiatrist, performs assessments, treatments and case management to include discharge planning for patients and families. Develops and implements treatment plans and discharge plans as well as provides direct therapeutic services including individual, family and group therapies. Provides referral to community resources and/or for hospitalization. Determines patient eligibility for resources.
Utilizes de-escalation and Crisis Prevention Institute (CPI) techniques. Serves as a resource to health care professionals regarding psychosocial problems and community resources. Performs any other job related duties assigned by Nurse Manager or Department Director.
If Social Worker
: MSW required. Licensed Independent Social Worker preferred. Two years of previous social services experience required.
Psychiatric Experience Preferred.
If Licensed Professional Counselor (LPC):
Masters in Counseling and SC licensure required.
