The Licensed Practical Nurse (LPN) functions under general supervision and direction of the Registered Nurses and Director of Nursing to:

• Assess the needs of patients and initiate, implement, and evaluate individualized plans of care.
• Relate effectively with other shifts for continuity of care.
• Maintain satisfactory relations with other departments and nursing units.
• Participate in all phases of education, maintenance of records, and skill development of personnel.

• High School Diploma or GED required.
• Graduate of an accredited practical or vocational nursing program required.
• Current, active LPN or LVN license in the state of Indiana required.
• Minimum of 1 year of nursing experience in a behavioral healthcare or acute care setting preferred.
