Community Health Network has been created by our neighbors, for our neighbors. Over 60 years later, “community” is still the heart of our organization. We provide our neighbors with the best care possible, backed by state‑of‑the‑art technology. We involve ourselves in the communities we serve through volunteer opportunities and benefits initiatives. We ensure our dedicated caregivers can learn and grow to stay at the top of their fields and to better serve our patients.
Above all, we provide exceptional care, simply delivered.
A Physical Therapist will interpret results of initial evaluation, determine appropriate patient goals, and develop treatment plans. We modify plans as needed and utilize appropriate treatment techniques. Physical therapists will also participate in teaching opportunities or clinical education of students as well as inter-disciplinary program planning.
