Job Summary &
Qualifications:
The Physical Therapist (PT) provides high‑quality, patient‑centered care by managing the therapeutic care process of an assigned group of patients in coordination with the physician and the care team. The PT completes patient assessments, contributes to individualized care plans, and delivers or directs patient physical therapy throughout the continuum of care.
Responsibilities:
- Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
- Assess and evaluate therapeutic/rehabilitative/functional status and collaborate in developing the total plan of care.
- Deliver purposeful and skilled therapeutic care using procedures and techniques to achieve results consistent with the physician’s plan of care.
- Instruct patients, families, and caregivers on therapies, exercises, therapeutic appliances, or prosthetic devices for use outside of therapy sessions.
- Prioritize caseload, determine daily staffing needs, and coordinate assignments and handoffs.
Qualifications:
- Licensed Physical Therapist (LPT).
- Driver’s License (DL).
- Bachelor’s degree.
