GENERAL STATEMENT OF DUTIES: Performs professional work involving managing staff nurses and nursing assistants in an assigned unit, and responsible for the quality of life and the nursing care provided to the residents in that unit on a 24 hour basis. Plans, coordinates, evaluates, communicates, and ensures quality patient care across all shifts. Maintains awareness of changes in each resident’s condition and needs on a daily basis.

Ensures documentation compliance for medical records maintained on unit. Works with and through other departments to achieve quality patient care and appropriate support and interventions for concerned family members.

Position carries a Blood and Body Fluid Exposure Risk Level I, and reports to the Director of Nursing.

Essential features / Responsibilities
  • Initiates, implements, updates, and evaluates 24 hour plan of care for each resident assigned to the unit in accordance with the multidisciplinary care planning goals and objectives.
  • Assures that nursing assistants’ daily assignments are based in accordance with established care plan; reviews assignment preparation delegated to evening and night charge nurses.
  • Responsible for the nursing assessment of new residents; reassesses the residents as needed, in accordance with care planning guidelines. Uses professional judgment and expertise in evaluating residents, staff and unit needs, and in working to improve care and the quality of life for the residents on his/her unit.
  • Supervises administration of medications and treatments performed by the staff nurse; administers medications and treatments in the absence of the staff.
  • Receives and transmits a change of shift report; holding the staff on the alternate shifts accountable for the delegated work.
  • Provides a daily report to staff members and provides assignments according to resident needs and doctor orders; informs staff of all significant changes occurring within the institution, and sees that these changes are implemented on the unit. Communicates regularly with doctors, staff and family members.
  • Supervises, evaluates, and works to upgrade the skill levels of assigned unit staff with supervisory assistance and support; completes disciplinary actions as needed with the guidance of nursing administration; maintains required documentation regarding staff counseling, on-the-job training, and evaluations.
  • Completes full rounds at least at the beginning and end of each shift; assures that residents requiring more frequent observation received required care; assists with direct physical care of residents when this is indicated as based on the resident’s needs or on staffing needs. Submits recommendations for improving care, changes in routines, procedures, etc. based upon current trends in nursing and professional observation.
  • Assures compliance with various documentation requirements involving resident and institutional records; monitors the accuracy of documentation provided by unit staff.
  • Participates in orienting new staff to unit and facility routines; prepares for and participates in resident care planning meetings; maintains awareness of environmental factors related to safety, infection control, and quality of life, and acts to correct deficiencies promptly when possible and/or reports conditions to the appropriate sources.
  • Implements and evaluates resident and family education as needed; attends required in-service programs and conferences intended to upgrade skills, and nursing administration meetings as required.
  • May participate in in-service education programs and on other committees and in studies involving other units as assigned.
  • Performs other related duties as assigned.
Required knowledge, skills and abilities
  • Considerable knowledge of geriatric nursing, laboratory values, pharmacology, and assessment.
  • Considerable knowledge of management principles in the areas of time management, delegation, prioritization, organization, supervision and disciplinary process.
  • Knowledge of care plan and staff assignment development.
  • Knowledge of infection control, safety, and quality assurance.
  • Skilled in effective communication, both verbally and in writing.
  • Skilled in venipuncture.
  • Ability to conduct unit meetings and clinical conferences.
  • Ability to effectively manage, delegate, problem solve, and motivate change.
  • Ability to conduct electrocardiogram testing and to utilize all the tools and equipment of geriatric nursing.
  • Ability to motivate and supervise staff.
  • Ability to physically perform all functions of geriatric care.
Minimum qualifications
  • Graduate of accredited college or hospital program with license as a registered nurse in State of Connecticut plus 5 years of RN experience including 2 years of supervisory experience in a SNF convalescent or nursing home, hospital or related health care facility.
Special necessary requirements
  • Must be CPR certified and trained in using electrocardiograms.
