• Ensures that all activities align with the vision of Elara Caring’s board of directors, executive team, and Home Health leadership.
• Maintains patient and staff privacy and confidentiality in accordance with HIPAA.
• Assesses the caseload of home health care patients and families to identify physical, psychosocial, and environmental needs, as evidenced by documentation, clinical records, Interdisciplinary Team reports, after-hours reports, and on-site evaluations.
• Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of care.
• Assesses patient needs and collects data on physical, psychological, social, and spiritual factors that may influence health status and incorporates that data into the plan of care.
• Makes initial evaluation and re-evaluates nursing needs during each visit; conducts follow-up visits as needed by patient, family, and facility.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other involved personnel.
• Revises the plan of care in response to identified issues and notifies the Clinical Supervisor, physician, facility, and other team members.

• Current, unrestricted state RN license
• 1+ year of clinical nursing experience
• Able and willing to travel within the branch/office coverage area
• Dependable vehicle, valid driver’s license, and current auto insurance
• Willingness to travel approximately 50%
• Associate or Bachelor’s Degree in Nursing preferred
• Previous Home Health or Hospice experience preferred
• Previous experience with Home Care Home Base preferred

You will report to the Clinical Manager or Clinical Supervisor.
