Job Overview

An experienced registered nurse is sought to join the diabetes team at the QEH site for a permanent position. The role involves providing specialist nursing advice and education for Type 1 and complex Type 2 Diabetes patients, supporting multidisciplinary care and delivering nurse‑led clinics.

Responsibilities
  • Work as a team player within the diabetes service, providing evidence‑based practice.
  • Support all aspects of diabetes management, reducing cardiovascular risk and working with the multidisciplinary team.
  • Deliver education and training to multi‑professionals, voluntary organisations and patients with diabetes.
  • Manage a caseload of patients, educating them towards self‑management, prevention, early detection and treatment of complications.
  • Collaborate across an integrated pathway of care to meet NICE guidance quality standards.
  • Ensure equitable access to investigations, treatment and follow‑up care with the nurse specialist team.
  • Develop best practice in line with NICE guidance and enhance optimal management of patients.
  • Collect data against NICE guidance, use audit to improve practice and evaluate services.
  • Implement recommendations from national audits and benchmarking.
  • Co‑ordinate specialty care within the Trust/CCG and serve as a point of contact for the multidisciplinary team.
  • Carry out diagnostic and therapeutic interventions as an independent practitioner, interpreting, writing reports and initiating further treatment.
  • Perform complex therapeutic/investigational procedures under indirect supervision, following safe practice and agreed protocols.
  • Demonstrate the ability to develop new skills and provide an expert, high‑quality nurse‑led service.
  • Initiate ongoing change and progression in the expanding role of a Clinical Nurse Specialist.
  • Develop interdisciplinary clinical management protocols and work within them.
  • Autonomously or collaboratively manage patient care within agreed clinical protocols and current practice.
  • Offer a supportive service to patients and carers, disseminating specialist knowledge and enabling adaptation to changes.
  • Maintain accurate and legible clinical and legal records in line with statutory and Trust policies.
  • Assess, develop, implement and evaluate programmes of care in acute and community settings.
  • Control patient admission via direct referral and with consultant guidance.
  • Participate in the reformulation of nursing aspects of the Divisional Business Plan and investigate incidents/complaints to initiate corrective action.
Qualifications & Skills
  • Experienced registered nurse with a commitment to making a difference.
  • Highly motivated with excellent communication skills.
  • Ability to provide specialist nursing advice in inpatient and outpatient settings.
  • Experience delivering education to healthcare professionals and patients.
  • Knowledge of NICE guidance and audit processes.
  • Capacity to work autonomously while seeking guidance when necessary.
