Discharge Team – AMU Discharge Coordinator
The Discharge Team are looking for a proactive person to join our dynamic team as an AMU Discharge Coordinator. The post holder will support the Complex Discharge Team and Senior Team Lead by ensuring safe and coordinated care and a high‑quality discharge service for all patients on the Hospital Site.
Roles & Responsibilities- Provide efficient administration to the Complex Discharge Team and Senior Team Lead.
- Actively manage a personal caseload of wards and coordinate complex discharges from Acute.
- Ensure all relevant paperwork (e.g. Discharge Passport, OOB referral, homeless referral) is completed by the appropriate persons for timely discharge.
- Maintain daily records of ECIST coding and pathways.
- Attend Bed Meetings and TOCC Meetings as required by the service.
- Provide education in discharge planning to members of the MDT or other professional groups.
- Act as the Trust expert in discharge planning; assist and support the MDT to drive and coordinate discharges.
- Attend daily whiteboard meetings and MDT meetings to ensure accurate EDDs and medically fit status for all patients.
- Actively monitor care pathways and lead discharge arrangements for all patients on designated wards.
- Liaise with patients and relatives on admission to discuss expectations, discharge dates and plans.
- Ensure the most appropriate option is identified and recommended by the MDT for each patient.
- Coordinate a seamless transfer of care between the primary and secondary sectors.
- Work 7‑day rotas and altered shift patterns, including weekends on a roster basis, as required.
- Escalate any delays in discharge to the Discharge Lead.
- Liaise with residential/nursing homes and provide regular updates to build positive relationships.
- Organise and coordinate family / best interest meetings, including venue sourcing and timing with the MDT.
- Maintain effective written and verbal communication with all members of the multidisciplinary team.
- Collaborate with multi‑agency representatives to reduce length of stay, minimise stranded patients and delayed transfers of care, and achieve performance targets related to discharge.
- Follow up any outstanding requests causing a delay in the discharge process for patients.
