- Assess patient needs and collect data on physical, psychological, social, and spiritual factors to inform the plan of care.
- Conduct initial and ongoing evaluations of nursing needs, and schedule follow‑up visits as required.
- Communicate significant findings, issues, and changes in health conditions or unsafe environments to the Clinical Supervisor, physician, facility, and other care team members.
- Revise the plan of care in response to identified issues and notify all relevant parties.
- Counsel, instruct, and involve patients, families, and caregivers in the Interdisciplinary Plan of Care.
- Teach hospice philosophy and pain/symptom management to staff in alternate care settings (nursing homes, hospitals, assisted living facilities, etc.).
- Collaborate and communicate with the Medical Director, attending physicians, branch leadership, and IDG.
- Perform other duties and projects as assigned.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
- Current state license as a Registered Nurse (RN).
- Minimum of 1 year of experience in a clinical care setting.
- Experience in hospice or home health environment is preferred.
- Strong nursing skills as defined by accepted nursing standards.
- Ability to handle multiple priorities, documentation requirements, and deadlines.
- Willing and able to travel up to 50% as needed.
- Capable of sitting, standing, bending, lifting, and moving intermittently and lifting 50–100 lbs.
- Valid driver’s license, insurance, and reliable transportation.
You will report to the Clinical Team Manager.
