At Step by Step Ministry, our mission is to provide tools and resources that lead to equity, empowerment, and social justice for reentry individuals and those who were previously incarcerated. We are seeking a Case Manager who is responsible for providing holistic and comprehensive case management services to diverse clients. She/he must have the ability to work independently, communicate effectively both verbally and in writing, and be able to maintain good working relationships with clients, employees, the public, and other volunteers.
It is essential that this person share the organization’s vision of strengthening individuals through social justice advocacy and holistic health and wellness.
- Conduct client intake assessments.
- Follow up with clients’ other service providers to document progress.
- Provide written client attendance reports for official documentation.
- Record case notes and data in the case management database for all client interactions.
- Connect clients with needed and available community resources.
- Conduct weekly case management and follow-up with clients to discuss their goals and provide counseling as needed.
- Advocate for clients within systems, such as the courts, justice systems, DCF, etc.
- Observe and manage the group dynamics in group classes.
- Participate in internal program enhancement as needed.
- Participate in staff and team meetings.
- Bachelor’s degree in social work, psychology, or other health care-related field (Master’s preferred).
- Relevant work experience with diverse or underserved communities, including internships.
- Other combinations of experience and education that meet the minimum requirements may be substituted.
- Bilingual abilities preferred.
- Personable and able to work nonjudgmentally within a person-centered service framework.
- Well-organized with detail-oriented, multi-tasking skills.
- Proficient use of Microsoft Word, Excel, PowerPoint, and non-profit CRMs.
- Experience with social justice organization or messaging.
