Full‑time | Seniority Level: Mid‑Senior. This role supports the health services for children in NYAP’s care and leads a team of Medical Coordinators.

- Provide children with a positive and appropriate medical environment consistent with NYAP’s mission.
- Oversee health services and coordinate timely, appropriate health services for children in care.
- Supervise other Medical Coordinators and provide staff training on medical, dental, and public health topics relevant to pediatrics, adolescent, and migrant health.
- Develop and maintain policies and procedures for confidential health record management, triage of health concerns, medical emergency response, environmental health and safety, and post‑release health care planning/care coordination.
- Ensure confidentiality of health records and adherence to NYAP, State, and Funder guidelines for protected health information (PHI).
- Communicate health needs to case managers and clinicians and support scheduling of health care appointments.
- Assist in transportation of children to and from health care appointments and in providing accompaniment during appointments.
- Obtain medical clearances from healthcare providers and prepare Treatment Authorization Requests (TARs) prior to appointments.
- Ensure qualified interpretation and translation services are in place.
- Participate in intakes and discharges, prepare medical documentation packages, and upload documents into health record systems.
- Collaborate with cross‑functional teams and external providers to ensure appropriate care for children, including PCU process completion.
- Report medical emergencies via SIR/CLE process in accordance with ORR/NYAP’s policies and procedures.
- Elevate medical concerns to supervisors, cross‑functional teams, healthcare providers, and the Division of Health (DoH) for consultation.
- Support foster parents by providing education on medical policies, answering questions, and supplying necessary authorization and .
- Maintain medicine cabinets and first aid kits with up‑to‑date OTC medications and supplies.
- Provide in‑service training and ongoing education to staff and foster parents on health‑related needs, medication administration rights, CPR, and First Aid.
- Maintain Red Cross CPR/First Aid trainer certifications and assist with auditing processes.
- Perform other duties as requested.
- Licensed Registered Nurse (RN) with a bachelor's degree or higher in a health‑related field.
- Bilingual fluency in English and Spanish is required.
- Minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care.
- Minimum of 1 year of experience working in child welfare with children and/or adolescents in a social service setting.
- Valid driver’s license, excellent driving record, safe and clean personal vehicle, and current automobile insurance.
- Ability to maintain compliance with agency‑required training programs.
- Proficient use of desktop/laptop computers, smartphones, tablets, printers, fax machines, photocopiers, and software including word processing, spreadsheets, and databases.
- Experience scheduling and documenting medical appointments.
- Understanding of immigrant population needs and ability to create a safe environment for program children.
- Demonstrated ability to maintain liaisons with community health providers and expand community health networks.
