Job Overview
:
The Behavioral Health Therapist will provide outpatient mental health services and interventions aimed at recovery and remission, including assessments, counseling, service plan development, referrals, and coordination of care. The Therapist will serve individuals and families to address social, emotional, and economic stresses associated with illness or injury, supporting an outcomes‑oriented patient care delivery system that places the patient at the center of all activities.
The Therapist is a core member of the outpatient collaborative Behavioral Health care team, working closely with the patient’s medical provider, psychiatric consultant, and primary care team.
- Coordinate mental and physical health care for patients on an assigned caseload; collaborate with medical providers and meet weekly with consulting psychiatrists to review progress.
- Screen and assess patients using PHQ‑9, GAD‑7, and other validated rating scales.
- Complete all clinical documentation—treatment plans, assessments, progress notes—in a timely and accurate manner.
- Provide psychoeducation and direct advice on depression, anxiety, and alcohol misuse, and communicate treatment plans to primary care providers.
- Use data from situational analyses, functional assessments, and the electronic medical record (EPIC/Connect Care®) to identify and prioritize patient support needs.
- Facilitate interdisciplinary care plan meetings to maintain rapport, monitor progress, and adjust plans; initiate referrals to community agencies as needed.
- Participate in education, case management, care coordination, and psychosocial referral and follow‑up; assist in preparing behavioral health reports and summaries.
Licensure:
Must hold LISW‑CP licensure level. Practice location:
Woodward Medical Center. The position utilizes the Collaborative Care Model, the most evidence‑based model for integrating behavioral health into primary and specialty care settings.
