The licensed clinician provides psychosocial evaluations, psychotherapeutic interventions, case management, and discharge planning. Focuses upon transitions in care for assigned patient populations. Provides screening, assessment, planning, problem resolution, resource management, social work services, therapy and crisis management as they relate to behavioral health care and discharge planning needs of the patient/family. Acts as a liaison to facilitate a comprehensive and individualized plan of care among physician, patient, family support systems, community agencies, facilities, and other entities within the patient system of care.
EssentialFunctions

Be the difference. - Develop individualised treatment plans encompassing discharge planning and follow up; including but not limited to facilitation of alternative placement, coordination of medical equipment, and case collaboration with referral resources to support transition/discharge to the next level of care.
- Prepare case history of patient's personal, social, and emotional development.
- Assist patients in the evaluation of their strengths and symptoms and the incorporation of these into personalised treatment goals.
- Provide therapeutic intervention with patient/family including individual, marital, parental, family, and group therapies.
- Provide education related to patient's diagnosis.
- Communicate patient and family issues to treatment team and act as the liaison between other active services and/or services involved in patient's discharge plan.
- Assist patients in locating and utilising community resources including legal, financial assistance, and other referral services.
- Strive to expand the variations of service provided by integrating community‑based programmes to provide home and community‑based treatment options for the identified patient/family.
- Responsible for clinical updates and advocate with 3rd party payers when required.
- Document all social service interventions and maintain patient and programme records in accordance with applicable standards and regulations.
- Monitor caseload and quality review to assure compliance for third‑party payers and regulatory agencies including, but not limited to, clinical documentation, chart review, completion of prior request and authorisations.
- Assume responsibilities for ensuring the individualised treatment plan is implemented with patient/family.
- Demonstrate a working knowledge of psychological, behavioural, physical, health, and medical sequelae of various disabilities and incorporate this knowledge into treatment planning.
- Demonstrate a working knowledge of various supportive, vocational, and community resources addressing and supporting recovery of various psychological, behavioural, and physical disabilities.
- Demonstrate knowledge of age‑appropriate interventions and cultural differences as applied to the recovery process.
- Coordinate outpatient scheduling including initiating the recertification of services.
- Apply facets of Mental Health First Aid during times of crisis to patients.
- Enhance professional growth and development through professional affiliations, participation in educational programmes, in‑service meetings, workshops and review of current literature.
- Participate in planning conferences and peer supervision, adhering to a professional code of ethics.
- Identify and work with potential community support systems such as neighbourhood networks and community organisations to support patient transition and discharge planning.
- Perform as Designated Examiner involving the commitment process, designated exam and testimony for probate courts.
- Provide oversight and supervision to students from local universities. Assist student interns from affiliated schools in the professional development of clinical skills.
