Job SummaryUnder the direct supervision of the Behavioral Health Director, the Behavioral Health Care Advocate will provide a range of services that link behavioral health patients with necessary supportive services. These may include internal services, as well as within the community. This individual will serve as point of contact and liaison for external collateral contacts, to assist patients and their NHFHS care team (primary care with integrated behavioral health) in achieving optimal physical and mental health.
Assists Behavioral Health team with ongoing tracking of patients who are receiving behavioral health services, including phone contact, assisting with scheduling, particularly for brief interventions and crisis interventions. Utilizes spreadsheet for patient due dates for treatment plans, and works with Quality Management Specialist regarding behavioral health department needs. This may include assisting LISWs in completing documents according to requirements. Care Advocate will work closely with Clinical Support Specialist regarding referrals (internally and externally as needed), as well as insurance authorizations.
Special focus will be on patients receiving Substance Use Disorder counseling, Alternative Therapies for Chronic Pain and monitoring timelines for patient and Behavioral Health department.
- Contact newly assigned behavioral health patients with reminder calls, explanation of expectations, providing exceptional patient-centered customer services and evaluating as well as resolving potential barriers to care. This may include linkage to interpreters and transportation.
- Coordinate services/supports as deemed necessary by PCP/LISW providers.
- Monitor the efficiency of the therapeutic treatment plan and utilize in-person visits and phone contact between scheduled LISW/Primary Care appointments to monitor changes in mood, symptoms or needs to meet behavioral health therapy goals.
- Review and reevaluate service plans on a regular basis, with LISW coordination.
- Help families and individuals to educate, encourage and assist them in understanding how to navigate services that will support their behavioral health needs, including mental health and/or substance use disorder treatment, both within NHFHS and outside agencies.
- Work with Emergency Department staff when patients need psychiatric care; substance abuse treatment/detox. May include transporting patients, if safety standards are met and as needed (in Agency vehicle).
- As required, work with PCP providers to coordinate immediate New Horizon Behavioral Health / LISW interventions during the patient’s visit. Be the point of contact for the BH Team.
- Attend interdisciplinary team meetings as appropriate, BH department meetings and agency staff meetings as scheduled.
- Document all contacts with patients in EHR and notify necessary parties.
- Hold all patient Protected Health Information (PHI) and other patient personal information and agency information in confidence.
- Actively participate and comply with all aspects of the NHFHS Corporate Compliance Program, follow the Program Code of Conduct and obey all relevant laws, statutes, regulations and requirements applicable to Medicaid, Medicare and other State and Federal health care programs.
- Participate in CQI, other internal committees, special projects/observances or activities that promote improvements in organizational performance and/or advance the mission, goals and objectives of New Horizon Family Health Services.
- Help families and individuals understand how to access the health care system.
- Perform any other duties as assigned by the Behavioral Health Director, CMO, CEO.
