We are currently seeking a Licensed Practical Nurse (LPN) to join our dedicated team and contribute to the well‑being of our residents.

As an LPN at Village Green, you will be responsible for providing direct patient care, assisting in the assessment and treatment of residents, and ensuring their comfort and well‑being. You will work closely with Registered Nurses (RNs), physicians, and other healthcare professionals to deliver high‑quality care and support for our residents.

• Provide direct care to residents, including administering medications and treatments as prescribed.

• Assist in the development and implementation of individualized care plans.

• Monitor and document patient progress, noting any changes in condition or behavior.

• Collaborate with RNs, physicians, and other healthcare professionals to ensure coordinated care.

• Provide education and emotional support to residents and their families.

• Assist in supervising nursing assistants (CNAs) in delivering patient care.

• Ensure adherence to facility policies, state regulations, and federal healthcare standards.

• Assist with patient admissions, discharges, and transfers.

• Maintain accurate, up‑to‑date patient records in compliance with HIPAA standards.

• Current Licensed Practical Nurse (LPN) license in the state of Ohio.

• Previous experience in a skilled nursing facility or long‑term care setting is preferred but not required.

• Strong knowledge of nursing practices, clinical procedures, and regulations.

• Excellent interpersonal and communication skills.

• Compassionate, patient‑centered approach to care.

• Ability to work in a fast‑paced, team‑oriented environment.

• Basic computer skills and familiarity with electronic health records (EHR).
