Overview

Position Title: Licensed Social Worker (LSW or MSW) |
Location: Western & Central Ohio |
Employment Type: Full-Time (40 Hours) |
Schedule: Monday–Friday, 8:00 AM–4:30 PM (occasional evenings/weekends as needed)

Position Summary

The Licensed Social Worker (LSW or MSW) provides psychosocial assessments, counseling, care planning, and resource coordination for patients and families in accordance with individualized plans of care. This role collaborates closely with the interdisciplinary team to support patients’ emotional, social, and end-of-life needs.

Qualifications, Skills & Requirements
  • Bachelor’s degree in social work (BSW) from a CSWE-accredited program required; MSW preferred
  • Active LSW or MSW licensure in the State of Ohio required
  • Minimum of two (2) years of clinical social work experience
  • Healthcare and/or hospice experience preferred
  • Valid driver’s license with ability to travel to patient locations (including inclement weather)
  • Strong customer service, interpersonal, and communication skills with excellent documentation, organizational, and time-management abilities
  • Ability to work independently and follow agency policies and procedures
Key Responsibilities
  • Conduct initial and ongoing psychosocial assessments for patients and families
  • Identify and address risk factors including abuse, neglect, suicide, and substance use
  • Provide therapeutic and supportive counseling, crisis intervention, and patient/family education
  • Assist with end-of-life planning, including advance directives and care coordination
  • Develop and participate in patient plans of care and interdisciplinary team meetings
  • Connect patients and families with appropriate community resources
  • Coordinate care transitions and ensure continuity of services
  • Complete accurate and timely documentation in compliance with regulatory and agency standards
  • Support bereavement services and other duties as assigned
