Job Overview
Registered Nurse, Baylor – Home Health vide personalized, one‑on‑one care in patients’ homes, developing and managing care plans while collaborating with physicians and interdisciplinary teams.
Responsibilities- Provide admission, case management, and skilled nursing visits for home health patients.
- Administer ongoing care and case management, ensuring follow‑up as directed by the Clinical Manager.
- Collaborate with physicians to develop and adjust initial treatment plans, manage care, educate patients under Clinical Manager supervision.
- Coordinate care with physical therapists, occupational therapists, home health aides, and external providers.
- Report patient condition, progress, and care plan updates to physicians and Clinical Manager.
- Assist patients and families in achieving optimal health outcomes.
- Supervise home health aides and personal care workers, provide written instructions and monitor documentation.
- Discharge patients with physician and Clinical Manager consultation, completing required documentation.
- Maintain thorough medical documentation, including conferences, contacts, medication orders, re‑certifications, progress updates, and plan changes.
- Diploma, Associate, or Bachelor’s Degree in Nursing.
- Minimum of one year nursing experience preferred; strong acute care background (ICU, ER, med‑surg) desired.
- Home health experience a plus.
- Current, unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
Pay Range: $45.00–$63.00 per visit/unit; $70,500–$96,900 annual base. 32 scheduled weekly hours. Comprehensive benefits include medical, dental, vision, 401(k), paid time off, disability, life insurance, and more.
