RN - Home Health

This is a potential 8-week contract position with the possibility of extension depending on census.

Monday - Friday, 8:00 AM - 5:00 PM

We are seeking a dedicated Home Health Registered Nurse Case Manager to provide exceptional patient care in the Greensboro/High Point area. This role involves case management and maintaining a case load while delivering home health services.

• Manage and coordinate patient care in a home health setting.
• Conduct patient assessments and develop care plans.
• Provide education and support to patients and families.
• Ensure compliance with health and safety regulations.
• Document patient progress and outcomes accurately.
• Travel to patients' homes as needed, utilizing your own vehicle.

• Minimum of 1 year of case management experience required.
• Must have a valid Registered Nurse license.
• Home Care Home Base charting experience is preferred.
• Travel experience required.
