The pay range for this position is $44,600.00/yr – $61,400.00/yr, based on skills and experience.

• Base pay: $44,600.00/yr – $61,400.00/yr
• $5,000 Sign‑On Bonus (subject to applicable conditions)
• Pay per visit/unit: $28.00 – $40.00

• Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act and Center Well Home Health policies.
• Deliver skilled nursing interventions for patients’ illnesses, rehabilitative needs, and preventive care, using a holistic, patient‑centered approach.
• Apply clinical practice standards to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Collaborate with the RN Supervisor on changes to the Plan of Treatment; accept and communicate authorized verbal orders from the physician.
• Implement a systematic, goal/outcome‑driven approach to the nursing care plan.
• Maintain timely contact with patients, physicians, clinical managers, and other team members regarding patient/family needs and status changes; participate in care coordination and discharge planning.
• Maintain the highest standards of professional conduct, protecting confidential information and sharing only when appropriate.
• Ensure proper supply, material, and equipment availability to safely implement the care plan.
• Prepare, submit, and maintain required documentation; document day services when shifts are rendered.

• Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of LPN/LVN experience in a clinical setting, preferably in home health or hospice.
