- Delivery of Patient Care
- Assumes responsibility for the coordination and delivery of patient/caregiver care.
- Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
- Develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved.
- Facilitates patient and caregiver independence to the extent possible.
- Utilizes VIA Health Partners patient-related protocols, standing orders and guidelines when providing hospice care.
- Assumes responsibility for conducting effective patient and caregiver education.
- Encourages and allows time for questions and consultation.
- Provides education to patients and families as needed.
- Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
- Maintains open lines of communication with supervisor and keeps supervisor apprised of complex patient/caregiver issues.
- Applies infection control principles and follows universal precautions in accordance with accepted professional practices.
- Interdisciplinary Group
- Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences.
- Establishes and maintains effective working relationships with team members.
- Identifies patient/caregiver needs and makes referrals to appropriate team members.
- Documentation
- Ensures accurate, complete, succinct, and timely clinical documentation according to VIA Health Partners guidelines and the requirements of licensure and certification standards.
- Professional and Personal Accountability
- Demonstrates competency in knowledge and skills.
- Maintains VIA Health Partners professional reputation and projects it.
- Assumes responsibility for professional development and staying current with trends in healthcare; incorporates new information into practice.
- Maintains current professional licensure, CPR certification and CEU requirements.
- Maintains productive and professional relations with external agencies and healthcare providers.
- Exhibits time management skills and the ability to plan the workday while remaining able to respond to crisis.
- Other duties as required
- IPU cleaning:
Cleans patient rooms as required per patient room cleaning procedure; ensures all areas of the facility are clean and stocked; maintains the family room; uses proper chemicals and PPE when cleaning.
- A Diploma or associate degree in nursing is required;
Bachelor’s Degree preferred. - A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- The Nurse Licensure Compact (NLC) allows a RN to have one multistate license in a home state to practice in other compact states.
- Current CPR required; CHPN preferred.
- Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life-limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical-surgical.
- Computer proficiency is required.
