Care Navigator – Authora Care Collective

Authora Care Collective is currently seeking a Care Navigator for the Guide Program (community‑based care). This full‑time hybrid position supports the Greensboro/Burlington areas with a High‑Point office base. Hours are Monday‑Friday 8:00 AM–5:00 PM, with occasional weekend community events as needed.

• Serve as the main point of contact for beneficiaries and caregivers.
• Conduct intake, assessments, and document progress notes in the EMR.
• Build strong relationships with beneficiaries and caregivers to promote engagement in medical care and medication adherence.
• Identify patient needs in the home or facility and coordinate appropriate resources.
• Collaborate closely with the Care Team clinical members for medical, practical, behavioral, or other complex issues.
• Maintain a person‑centered care plan and document care outcomes in the EMR system.
• Communicate effectively in writing and verbally with Authora Care teams, community physicians, and partner organizations.

• Graduate of a four‑year social work or human service program (psychology, sociology, human relations).
• Ability to establish and maintain effective working relationships with patients, other employees, and the public.
• Experience with Microsoft Word, Excel, and HCHB (Homecare Homebase).
• Preferred experience working with older adults, caregivers, and seniors with dementia‑related issues.
• Current CPR certification.
• Valid state‑issued driver’s license, automobile liability insurance, and reliable transportation.
