Position Summary

Under the supervision of the Team Lead, the CST Qualified Professional provides therapeutic services, crisis intervention, and care coordination for members and their families. This position requires strong clinical judgment, excellent documentation skills, and the ability to collaborate with multidisciplinary teams while supporting members along the continuum of care.

Essential Duties & Responsibilities
  • Provide direct therapeutic services and interventions to CST members.
  • Implement therapeutic activities and community-based support schedules.
  • Effectively manage crisis situations and assist with developing member-specific crisis plans.
  • Maintain accurate and timely clinical documentation, adhering to regulatory standards.
  • Participate in developing and updating Person-Centered Plans, TARs, authorizations, and NCTOPPS.
  • Attend required supervision sessions (individual and group) twice each month.
  • Protect confidentiality in accordance with HIPAA, agency policies, and ethical standards.
  • Coordinate transitional planning and ensure linkage to community resources and natural supports.
  • Participate weekly in CST clinical care meetings and treatment planning.
  • Provide reports to Licensed Professionals and/or Quality Assurance Director as required.
  • Organize and prioritize job duties with minimal supervision while meeting deadlines.
  • Participate in tasks related to accreditation and quality improvement.
  • Build and maintain positive, professional relationships with members, families, and stakeholders.
  • Work effectively with diverse cultural and multilingual populations.
  • Remain flexible with schedule adjustments as needed.
  • Consult with medical and non-medical providers to support person-centered planning.
  • Arrange psychological and psychiatric evaluations when required.
  • Monitor member progress and document effectiveness of interventions.
  • Provide psychoeducation as outlined in the Person-Centered Plan.
  • Perform other related duties as assigned by the Licensed Professional or Leadership Team.
Education & Qualifications

Applicants must meet Qualified Professional criteria:

  • Bachelor’s degree in a human services field + 2 years post-graduate MH/SU experience with adults
  • Bachelor’s degree in any field + 4 years post-graduate MH/SU experience with adults
  • Master’s degree in a human services field + 1 year post-graduate MH/SU experience with adults
  • Master’s degree in any field + 2 years post-graduate MH/SU experience with adults

Required:

  • Valid driver’s license and reliable transportation
  • Strong interpersonal and communication skills
Physical Demands
  • Regular walking, standing, and stooping
  • Occasional lifting, carrying, pushing/pulling up to 25 pounds
  • Regular driving for community-based visits
Working Conditions

This is a community-based role. Work may occur in:

  • Clinics and community health centers
  • Schools and shelters
  • Members’ homes
  • Office and field settings
